Ordering samples

Forma will be pleased to send you samples at the retail price. Shipping is billed to the client. For any subsequent order, Forma will credit you for the value of the samples. Moreover, you will be credited for the return shipping if the order is worth more than $500. All returns must be unworn, in the original packaging. Samples must be returned 10 days after receipt. It is the customer’s responsibility to return the merchandise in its original packaging. Fees may apply if products are not returned in their original condition. Sample shipping charges may apply at any time. A 15% restocking fee may be applied.
